Hospital billing is also complex and error-prone. A single hospital stay can generate dozens of separate charges for room and board, nursing care, medications, laboratory tests, imaging, surgical supplies, and professional fees. Each of these charges is an opportunity for errors.

Understanding that hospital bills are negotiable is the first step toward reducing what you owe. Most hospitals would rather receive a partial payment than risk non-payment, and many have financial assistance programs that can significantly reduce or eliminate your bill.

Reviewing Your Hospital Bill

Request an itemized bill that lists every charge individually. The summary bill you receive initially does not provide enough detail to identify errors. An itemized bill shows the date, description, and cost of each service, supply, and medication.

Review each charge carefully. Look for duplicate charges, services you did not receive, incorrect quantities, and charges for items that should have been included in the room rate. Common errors include double-charging for medications, charging for unused supplies, and billing for services provided by in-network physicians at out-of-network rates.

If you find errors, contact the hospital billing department immediately. Provide specific details about each error and request a corrected bill. Most hospitals will investigate and correct genuine errors, potentially reducing your bill by hundreds or thousands of dollars.

Negotiating Your Hospital Bill

Start by asking about the hospital financial assistance program. Nonprofit hospitals are required by law to offer financial assistance to qualifying patients, and many for-profit hospitals offer similar programs. Income eligibility varies by hospital, but many offer assistance to households earning up to 300% to 400% of the federal poverty level.

If you do not qualify for financial assistance, negotiate directly with the billing department. Offer to pay a lump sum for less than the full amount. A reasonable starting offer is 30% to 50% of the total bill. Be prepared to negotiate, and always get any settlement agreement in writing.

Ask about the self-pay discount. Many hospitals offer significant discounts to uninsured patients or patients with high-deductible plans who pay promptly. The self-pay discount can range from 20% to 60% off the chargemaster price.

What If the Bill Goes to Collections

If your hospital bill goes unpaid, it may be sent to a collection agency. When this happens, you have the right to request debt validation. Send a debt validation letter within 30 days of the collection agency first contact to verify the debt and check for errors.

Medical debts in collections are subject to special credit reporting rules. As of 2023, credit bureaus no longer report medical debts under $500, and paid medical collections are no longer reported on credit reports. Additionally, medical debts cannot appear on your credit report until they have been in collections for at least one year.

Negotiate with the collection agency just as you would with the hospital. Collection agencies typically purchase medical debts for pennies on the dollar, so there is often significant room for settlement. Start with an offer of 30% to 40% of the total amount and negotiate from there.
